Medical and health care services:
Assisted living residences ("ALRs") are a
residential alternative for senior individuals, generally of caring for
themselves, who no longer feel comfortable or safe living alone or with a
spouse in their own apartment or home. ALRs were developed to increase
the spectrum of living alternatives for seniors, in support of the goal of
aging in place. ALRs can delay, and even eliminate altogether, the
movement of the elderly to nursing homes, where costs are higher and
quality of life issues more pronounced.
ALRs are not medical or nursing facilities providing
ongoing medical treatment or skilled nursing services, and are not
regulated as such. Accordingly, state laws limit ALRs in one way or
another from admitting or retaining individuals who require 24-hour
skilled nursing supervision for more than a specified number of days,
usually 90 to 120 days in any twelve month period. Other states have
created specific levels or types of ALRs depending on the ADL needs of
residents or left the level of services more flexible and directly
regulated the provider of services separately from the ALR.
As ALRs were envisioned, there are circumstances under
which residents might need to receive skilled nursing care in the ALR,
just as the resident would need this care if they were in their home. The
extent to which a residence who needs skilled nursing service can be
admitted or retained has been a point of concern in some states.
Where residents in assisted living facilities need
specialized, skilled medical care for short periods of time, there are
several ways that assisted living facilities accommodate their residents
needs. Arrangements to provide this care are either done informally
through referral, or more formally, as having a licensed nurse on staff or
contracting with a home health agency to be the provider when care is
needed.
